Community Health Plan of Washington (CHPW) members can get gift card rewards for prenatal, postpartum, and well-child checkups through our ChildrenFirst™ Program. Your gift card rewards can be used at Amazon, Safeway/Albertsons, or Target.
Regular doctor visits and prenatal checkups are covered through CHPW. Scheduling these visits today can make a major difference to your child’s health for years to come:
- Prenatal visits keep you and your baby safer by catching potential problems early on.
- Giving birth can change your body and have long-term effects on your health. Postpartum visits make sure you’re healthy after delivery.
- Regular well-child checkups give you a chance to get to know your doctor. You can discuss concerns you have, learn what to expect as your child grows, and check for preventable health issues.
$100 in gift cards for prenatal visits
Prenatal care is critical for a healthy you and healthy baby. You can get up to $100 in gift cards if you see your doctor during your pregnancy.
$50 gift card for postpartum checkup
Get a $50 gift card when you visit the doctor 1 to 12 weeks after giving birth.
$20 gift cards for every checkup
If your child is a CHPW member, you can get a $20 gift card for every time your child goes in for a well-child checkup until they are 18. That’s 27 checkups and 27 gift cards for every child, or a benefit of up to $540.

How do I know that my rewards form has been received?
You will get a pop-up and email confirmation message once you submit the online form.
How long does it take to get my rewards card?
Two to three weeks after we receive your request, if it is approved.
What if I don’t get my rewards card after 3 weeks?
Call Customer Service at 1-800-440-1561 (TTY: 711), 8am to 5pm, Monday – Friday. Or email [email protected].
What if I change my mind after submitting a request and would like a physical card instead of a digital card?
Unfortunately, once a reward card request has been submitted, we are unable to convert physical card requests to digital card requests, or vice versa.
Can I swap my gift card reward for another one? (e.g. swap a Target card for an Amazon one)
Once a gift card has been sent, we cannot swap merchants.
How long are digital codes and/or physical gift cards active? Do they expire?
There is no expiration on funds for digital gift card codes. Physical cards may have an expiration date – if the physical card does expire, members can call the number on the back to request a new card be issued so they can use the funds.
Can I use my digital gift card at physical store locations? Can I use my physical gift card online?
Digital gift card emails may include a barcode that can be scanned at a physical store; this varies by merchant. You should check the terms and conditions for any restrictions.
Physical cards are redeemable at the merchant’s website as well as their physical stores.
Do my gift card rewards ever expire?
Digital gift cards do not expire. Physical cards may have an expiration date — you should check the back of their card for full terms and/or contact information to request a new card if theirs has expired.
