HealthMAPS Patient Information Portal
CHPW’s HealthMAPS patient information portal lets you enter and view member claims, check eligibility, view and report a member’s other health insurance (OHI), and more.
CHPW is implementing Multi-Factor Authentication for all of our provider-facing portals, including HealthMAPS. This enhances the safety and security of CHPW’s provider and membership data.
OneHealthPort already offers Multi-Factor Authentication for Community Health Plan of Washington.
Beginning September 27, if you try to create a new HealthMAPS account or log in to HealthMAPS directly, the system will redirect you to OneHealthPort. You will then need to log in to OneHealthPort to access HealthMAPS. Please see the HealthMAPS FAQs and Training materials below for more information.

How to create a HealthMAPS accountYou can register for a HealthMAPS account online. You will need to know your Billing Tax ID number(s) in order to do so.

JIVA Care Management Portal
We prefer that you submit prior authorizations through our JIVA Care Management Portal. By using the portal, you can check eligibility and authorization status, print approval letters, and submit requests online 24/7. JIVA allows expedited processing for authorizations and referrals.

OneHealthPort Portal
Use the OneHealthPort portal to view and exchange information across Washington’s health care organizations. Please continue to use the JIVA Care Management Portal to submit and view prior authorization requests, referral requests, and inpatient notifications.
