CHPW also serves Medicare and Dual-Eligible (SNP) patients through our Medicare Advantage plans
Older patients may qualify for a Medicare plan through CHPW. Our Medicare Advantage plans include a dual-eligible Apple Health-Medicare (SNP) plan.
