Get a $50 gift card after your baby is born
There’s no busier time than right after you give birth. Community Health Plan of Washington (CHPW) wants to reward you for taking care of yourself.
We offer our members the reward of a $50 gift card to spend at Amazon, Safeway, or Target. Just visit your doctor for a postpartum checkup between 7 and 84 days (1 and 12 weeks) after giving birth.
Get a $50 gift card for Amazon, Safeway, or Target when you visit your doctor within 1 to 12 weeks after giving birth.
How it works
- Be a CHPW member.
- Complete one postpartum visit.
- Fill out the Online Reward Submission Form within 180 days of that visit to qualify.
- You will receive a $50 gift card in the mail within two to three weeks.
