Community Health Plan of Washington (CHPW) uses a list of approved drugs. This is called a “formulary” or a “preferred drug list.” Your doctor or pharmacist should prescribe medications to you that are preferred.
You can call us anytime and ask for:
- A hard copy of the preferred drug list
- Information about the group of providers and pharmacists who created the formulary
- A hard copy of the policy that explains how we decide what drugs are covered
- How to ask for authorization of a drug that is not on the formulary

COVID-19 Vaccines
The Pfizer BioNTech, Moderna, and Johnson & Johnson COVID-19 vaccines are covered as part of CHPW’s medication benefit at any location they are offered.
