Use the following codes for the situation that applies to your patient. If none of the situations apply, please fill out a Chronic Opioid Attestation form and fax it to Express Scripts at 1-877-251-5896.
Patient’s prescriber has indicated “Exempt” on the prescription (acute use only):
- Enter PA Type: 01
- Enter PA Number: 85000000541
Patient has a diagnosis indicating cancer treatment, hospice care, palliative care, or other end of life care:
- Enter PA Type: 01
- Enter PA Number: 85000000540
If none of the above situations apply, please complete a Chronic Opioid Attestation form and fax it to Express Scripts at 1-877-251-5896.
