No missed checkups means earning gift card rewards along the way. Eligible children may earn up to $540 per child in rewards from age 0 to 18 by completing qualifying well-child checkups.*

Is your family growing?
CHPW’s Healthy You, Healthy Baby program provides individual support before, during, and after pregnancy. Plus, if you’re pregnant or recently gave birth, you may be eligible to earn rewards for completing qualifying prenatal and postpartum checkups through the ChildrenFirst Prenatal and Postpartum Rewards Program.
How to earn and claim rewards
After your child visits their doctor for a well-child checkup, fill out a short form and we will send you a $20* gift card by mail or email. Repeat after each visit!

Recently complete a well-child visit?
To complete the form to claim your reward, you will need to know the date of the appointment, your child’s doctor’s name, and your child’s CHPW member ID number (It’s on their ID card).
Claim your reward within 180 days after each well-child visit to qualify.
*Total of $540 calculated from all well-child checkups from newborn until age 18. Apple Health members can earn up to $200 in total rewards per year.
About well-child checkups
Well-child visits are no-cost, routine checkups with your child’s doctor that support your child’s health and you as a parent. These visits help the doctor monitor your child’s health, growth, and development so they can have the best chance of a healthy future.

How much do well-child checkups cost?
Your cost is $0. As a CHPW Apple Health member, and with most health plans, you pay nothing for well-child visits and other types of preventive care when you see an in-network provider.
How many checkups do newborns get?
Most babies have at least 8 checkups in their first 18 months. A newborn’s first well-child checkup should happen at 1 week and then at months 1, 2 , 4, 6, 9, 12, 15, and 18. After 3 years of age, well-child checkups are only once per year.
What happens at a well-child checkup?
At each well-child visit, your child’s doctor will:
- Check how they are growing and developing.
- Offer recommended immunizations and screenings.
- Give guidance on the next stage for your child or teen, and what to expect.
- Ask questions about health history and day-to-day life.
- Talk about your concerns and answer your questions.
For newborns and toddlers, your child’s doctor may also ask about different things they are doing at home, to help track their development.

Vaccines and Immunizations
Immunizations help protect your little ones, you, and your community from diseases that can cause serious illness.
During your well-child visits, your child’s doctor will give you details about what each immunization guards against and any side effects.

In Washington State, children entering school, childcare, or other early learning programs are required to have certain immunizations before they can start.

Frequently asked questions
Where can I find my child’s CHPW member ID number?
Your child’s CHPW member ID will be different than yours, even if you have a CHPW health plan too. They will have their own CHPW member ID card and number. This number is not the same as your ProviderOne ID.
You can find their CHPW member ID number a few places:
- Child’s physical CHPW member ID card. It’s labeled “ID”.
- myCHPW member portal. If you already created an account for your child’s CHPW Apple Health plan, you can sign in to and see it there.
- CHPW Customer Service can also help you get it. Call 1-800-440-1561 (TTY: 711) from 8 a.m. to 5 p.m., Monday to Friday.
Tip: When entering your child’s ID number in the claim rewards form, you will need to include 10 digits with no spaces. In the form, under CHPW Member ID Number, include (or add) 01 at the end.
Can I get help with transportation to and from well-child visits?
Yes. Apple Health covers nonemergency medical transportation to and from covered services, including well-child checkups, if you’re eligible.

Who is eligible for gift card rewards for well-child visits?
CHPW’s Well-Child Program is open to parents or guardians of CHPW Apple Health members. As the parent or guardian, you do not need to be a member too.
To be eligible to earn rewards for well-child visits:
- Complete a recommended well-child checkup.
- Child must be a member on the well-child visit date and age 18 years or younger.
- Claim your reward within 180 days of the visit by completing the online form.
Note, eligible CHPW Apple Health members can earn up to $200 per year, per member.
