The Washington State Department of Health (DOH) wants to hear from you about what families need to thrive, especially families of children and youth with special health care needs. As part of a larger Maternal and Child Health Needs Assessment, we are asking families and community members across the state to take a short Discovery Survey about what families need to live their fullest lives and unmet needs.
The survey is open to anyone in the community including youth, families, and the providers that serve them. The Discovery Survey is available in English, Spanish, Vietnamese, Korean, Russian, Ukrainian, Arabic, Punjabi, Khmer, Somali, and Tagalog.
The DOH wants to make sure that the needs of Children and Youth with Special Health Care needs and their families are well represented!
Posters with QR codes are also available if you have a community space that may be willing to help get the word out to members of your community.
