Community Health Plan of Washington is excited to offer a new avenue to request changes to your existing EFT account or to sign up for EFT! This option is more secure and will help streamline the process to complete your request.
Starting May 1, 2024, you can submit EFT requests using CHPW’s HealthMAPS portal.
- First you’ll need to go to CHPW’s OneHealthPort page.
- Click “HealthMAPS Login.”
- Once you’re logged in, choose “EFT Enrollment and Updates”.
- Simply complete the online form to request changes to your existing EFT account or to file a new application for EFT.
We’ll process your request and you’ll receive notification when your application is reviewed.
