Preventive care is an important part of staying healthy and catching problems before they become serious. When you look after your health, we think that deserves a reward!

As a Community Health Plan of Washington (CHPW) member, you can earn gift cards through our MemberFirst™ Rewards Program for completing certain preventive care visits or screenings. Choose gift cards from Amazon, Safeway/Albertsons, or Target.

CHPW covers preventive care services at no cost to you. So, make preventive care a regular part of your health routine. Talk with your doctor to see what type of screening is right for you.

As a CHPW Apple Health (Medicaid) member, you can earn gift card rewards for:

  • Breast cancer screenings – Mammograms are the main test to screen for breast cancer and catch potential problems early on.
  • Colorectal cancer screenings – These are tests done at home or in a clinic or hospital to look for anything unusual in your lower digestive system.

After you complete your visit or test, submit a MemberFirst™ reward form (located below) to get your gift card.

$50 gift card for breast cancer screenings

A mammogram is an X-ray image of the breast used to screen for and detect breast cancer. It is the most common type of breast cancer screening.

You can get a $50 gift card if you complete your mammogram screening.

Mammogram: This is the most common test to screen for breast cancer. It can detect tumors that are too small to feel and can find cancer before symptoms appear.

Screening guidelines: The US Preventive Services Task Force recommends that women who are 40 to 74 years old and are at average risk for breast cancer get a mammogram every 2 years.

To receive your reward:

  1. Complete a mammogram as recommended by your doctor.
  2. Then fill out the online form to receive your $50 reward.

$25 gift card

$25 gift card for colorectal cancer screenings

There are different types of colorectal cancer screening tests including some you can do in the comfort of your own home. Experts recommend most people start to get screened at age 45.

Talk to your doctor about which screening is right for you and how often you should get it done.

You can get a $25 gift card if you complete an at-home test or a colonoscopy.

Some of the most common colorectal cancer screening tests include:

FIT Test (Fecal Immunochemical Test): At-home test. Checks for hidden blood in the stool. You get a special kit from your doctor or health care provider. You use the kit to collect a small sample of your stool. Then, you follow the instructions in the kit to send the sample to a lab for analysis. This test is usually done every year.

Cologuard Test: At-home test. Checks for hidden blood and DNA changes in the stool. You get a special kit from your doctor or health care provider. You use the kit to collect a small sample of your stool. Then, you follow the instructions in the kit to send the sample to a lab for analysis. This test is usually done every three years.

Guaiac-based fecal occult blood test (gFOBT): At-home test. Checks for blood in the stool. You receive a test kit from your doctor or health care provider. You use the kit to collect a small amount of your stool. Then, you return the test kit to the doctor or a lab for analysis. This test is usually done every year.

Colonoscopy: Usually done at a clinic, outpatient facility, hospital or clinic. A doctor uses a long, flexible tube with a camera to look inside your large intestine. It’s recommended that most people get a colonoscopy every 10 years starting at age 45. However, your doctor may suggest more frequent screenings depending on your health history.

Sigmoidoscopy: Usually done at a clinic, outpatient facility, hospital, or doctor’s office. A doctor uses a long, flexible tube with a camera to look inside the lower part of your colon. This test is recommended every five years starting at age 45. However, your doctor may suggest more frequent screenings depending on your health history.

Eligibility for MemberFirst™ Rewards

Breast Cancer Screening:

  • Must currently be enrolled in CHPW Apple Health and must also have been enrolled at the time of screening
  • Must be age 40 or older

Colorectal Cancer Screening:

  • Must currently be enrolled in CHPW Apple Health and must also have been enrolled at the time of screening
  • Must be age 40 or older

Rewards for each screening type are limited to one every calendar year.

How it works

It’s easy as 1-2-3!

  1. Complete your preventive care screening visit or test.
  2. Fill out the Online Reward Submission Form below for each screening visit or test within 180 days to qualify. You will need your 10-digit Member ID number (found on your CHPW Member ID card) and your date of visit or test.
  3. You will receive your gift card(s) by mail or email within 2-3 weeks.

Frequently Asked Questions (FAQ)

What kind of rewards can I earn?

Rewards vary based on the type of screening. CHPW Apple Health members can earn gift cards for breast cancer and colorectal cancer screenings.

They can also earn gift card rewards for prenatal, postpartum, and well-child checkups through our ChildrenFirst™ Program.

Members can receive up to $200 in total rewards per year.

Who is eligible for the rewards?

CHPW Apple Health members who meet the age requirements and who complete the recommended screenings are eligible.

How often should I get screened?

Your doctor will recommend what type of screening you should get and how often you should get it. They will consider your age and health history. For more information on different types of health screenings, read our CHPWConnections Be Well blog post.

Is there a cost for the screenings?

Most preventive screenings are covered by your health plan at no cost to you. Check with your provider or call CHPW Customer Service at 1-800-440-1561 (TTY: 711), 8 a.m. to 5 p.m., Monday – Friday for details.

How do I know that my rewards submission form has been received?

You will get a pop-up and email confirmation message once you submit the online form.

How long does it take to get my gift card reward?

Once we receive your request, it will take 2-3 weeks to receive your gift card, if your request is approved.

What if I don’t get my gift card after 3 weeks?

Email us at [email protected]. Or call Customer Service at 1-800-440-1561 (TTY: 711), 8 a.m. to 5 p.m., Monday – Friday.

What if I change my mind after submitting a request and would like a physical card instead of a digital card?

Unfortunately, once a reward card request has been submitted, we cannot change digital card requests to physical card requests, or vice versa.

Can I swap my gift card reward for another one? (e.g. swap a Target card for an Amazon one)

Once a gift card has been sent, we cannot swap merchants.

Do my gift card rewards ever expire?

Digital gift cards do not expire. Physical cards may have an expiration date. If the physical card does expire, members can call the number on the back to request a new card so they can use the funds.

Can I use my digital gift card at physical store locations? Can I use my physical gift card online?

Digital gift card emails may include a barcode that can be scanned at a physical store; this varies by merchant.

You should check the Terms and Conditions for any restrictions. Physical cards are redeemable at the merchant’s website as well as their physical stores.
