In the middle of sports practices and book reports, your child’s regular checkups and immunizations are an important part of helping them grow up healthy.

Now, we make it a little more rewarding to get this job done.  If your child is a Community Health Plan of Washington (CHPW) member, you can get a $20 gift card every time they go in for a checkup. That’s 27 checkups and 27 gift cards for every child! Or a total benefit of up to $540.

How it works

• Bring your child in for a checkup. If you’re not sure when to schedule checkups, see our Well-Child Reward checkup calendar at the link below.
• Fill out the Online Reward Submission Form within 180 days of that checkup to qualify.
• You will receive a $20 gift card in the mail.
• Repeat the steps above for your child’s next checkup!

Are you eligible for gift cards through CHPW’s Well-Child program?

1. Child must be a CHPW member.
2. Child must be 18 years old or younger.
3. Child must complete a well-child visit as a member.
