Be Well: You Had Questions, We Have Answers

We recently sent out a survey to our Apple Health (Medicaid) members asking for feedback on our Be Well series of articles about health tips, plan benefits, and more. We received tons of responses, so thank you for your thoughts! Many of you also had questions, so we thought we’d take the time to answer some of the more common ones here.

How do I find a doctor, specialist, acupuncturist, massage practitioner, etc., who is covered by my Apple Health plan?

The easiest way to find a doctor or specialist who accepts CHPW Apple Health (Medicaid) insurance is to use our Find a Doctor online tool. You can search by ZIP Code/City/County and Provider Type.

There are also links on the webpage to help you find a dentist, vision provider, or alternative treatment provider (licensed acupuncturist, chiropractor, or massage therapist).

Still can’t find what you’re looking for? Contact our Customer Service team at 1-800-440-1561 (TTY: 711), 8 a.m. – 5 p.m., Monday through Friday. They can help you find a provider and schedule an appointment.

How do I get or print a Member ID Card?

CHPW sends you a Member ID card when you first join our plan, or when you get a new Primary Care Provider.

If you don’t have your card or have lost it, you can log into CHPW Member Center to download and/or print a copy. If you haven’t set up a Member Center account, just follow the easy instructions on the page to Register.

You can also call Customer Service at 1-800-440-1561 (TTY: 711), 8 a.m. – 5 p.m., Monday through Friday. They can mail a new card to you.

Remember to bring your card to your health appointments.

Does my insurance cover vision services? Where can I go to get glasses?

CHPW covers regular eye exams for adults and children. That means you don’t have to pay anything for them. Eyeglasses for children ages 20 and younger are covered directly by Apple Health using your ProviderOne services card.

CHPW also offers a free eyeglasses benefit for adults (21 and older). Members can get a free pair of glasses (frames and basic lenses) through the VSP network every 24 months. Low-cost upgrades and extras like progressive lenses, tinted lenses, and UV coating are also available; for these, you pay out of pocket.

Note that each provider has their own selection of eyeglasses to choose from. If you have questions or need help picking a provider, Customer Service can help. Call 1-800-440-1561 (TTY: 711), 8 a.m. to 5 p.m., Monday through Friday.

Does my plan cover dental care? How do I find a dentist?

Dental services for children ages 20 and younger are covered directly by Apple Health using your ProviderOne services card. Covered dental services include routine exams, cleaning, X-rays, and fillings.

Some dental services for adults (21 and older) are also covered directly by Apple Health using your ProviderOne services card. These include routine exams, cleaning, X-rays, and fillings. Some adult dental services that are not covered include bridges, porcelain crowns, and orthodontics.

You can also call the Health Care Authority at 1-800-562-3022 or visit their website for more information about Apple Health dental benefits.

Remember, dental care is an important part of your overall health, so don’t put off those dental checkups.

Who can I speak with about my benefits? It would be helpful to have a one-on-one conversation with a human being so I can understand what and where my resources for health are.
