If you don’t have your CHPW member ID card, our Customer Service Team can help. Call 1-800-440-1561 (TTY: 711) Monday to Friday 8 a.m. to 5 p.m.

Why isn’t my member ID working?

There could be a few reasons why your ID isn’t working during sign up:

  • Are you looking at a white card or a blue card (ProviderOne Services)? Apple Health members have two ID cards. You will need to use the ID number on your white CHPW member ID card to register for myCHPW.
  • Does your CHPW member ID card show 8 digits or 10 digits? For CHPW Apple Health plans, if your CHPW member ID shows only 8 digits, try adding 01 at the end.
  • Did you recently change your CHPW health plan? If you recently had an Individual & Family Cascade Select plan and became eligible for Apple Health, your CHPW member ID will change. You will get a new CHPW member ID card in the mail.
  • Is your health plan or coverage active? To register, your coverage must be active, and you must still be eligible for Apple Health. Not sure? To check if your coverage is active, call our Customer Service Team 1-800-440-1561 (TTY: 711).
  • Are you registering for yourself or someone else in your family? Each person has their own ID card and ID number. If you are creating an account to see your child’s CHPW Apple Health coverage, you will need to use their CHPW member ID (10 digits, ends in 01).
