You are at the center of your health. At Community Health Plan of Washington (CHPW), we listen and center care around you. We want to make sure you can always get the care and services you need, whenever you need them. That’s why we are teaming up with Landmark Health, a provider of in-home medical care, to bring doctors to you.
That’s right, doctor house calls are back! CHPW works with Landmark to make house calls – at no cost to you. This is in addition to the care you get from your regular doctors and specialists. Landmark updates your primary care doctor after each in-home visit.
What is Landmark Health?
|Landmark is a health care provider group that partners with CHPW to make house calls to CHPW members with multiple chronic conditions. Landmark’s providers – doctors, nurse practitioners, and physician assistants – come to your home to help keep you well.|
What services does Landmark offer?
Landmark providers are available any time of the day or night. That means you can get help even when your regular doctor’s office is closed. Landmark does:

|Urgent care visits|
|Hospital post-discharge visits|
|24/7 telephone support|
Who are Landmark’s providers?
Landmark’s providers are a team of health care professionals including:
- Doctors, nurse practitioners, and physician assistants
- Behavioral health providers
- Nurse care managers
- Social workers
I already have a primary care doctor. Can I still use Landmark?
Yes! Landmark does not change your current CHPW health plan or benefits in any way. You keep your primary care provider and can still access the physicians and providers within your CHPW network. Landmark updates your regular doctor after each in-home visit.
What does it cost?
There is no additional cost or copay for eligible CHPW members. The program is voluntary; you don’t have to take part if you don’t want to. However, if you are eligible, Landmark services can help you stay healthy.
