1095-B tax form and Apple Health

The 1095-B is a tax form verifying who in an individual’s household has received Apple Health coverage for one or more months of the previous tax year. This form is informational only and not required to complete a federal tax return.

Individuals can access their 1095-B tax form by logging into their Washington Healthplanfinder account.

If you do not have a Washington Healthplanfinder account, you can create one on the Washington Healthplanfinder webpage.

Individuals who are age 65 or older, have blindness, or a disability, or are receiving long-term services and supports can request their copy by using the Washington Apple Health web form (select 1095-B from the drop-down menu), or call the Medical Assistance Customer Service Center at 1-800-562-3022.
