Community Health Plan of Washington (the Plan) is committed to protecting the privacy and security of the users of our web site. Protecting your privacy on the Internet is important to us.
Purpose of this web privacy statement
In this privacy statement we tell you:
- What information our web site gathers.
- How we might use the information the site gathers.
- Whether we will disclose the information to anyone.

If you follow links from our site to outside web sites, please read the privacy policies of those sites. It is important to understand how those sites may use your personal information.
How we protect the privacy of information that identifies you
Personal identification information:
- When you browse our web site, the site gathers no personally identifiable information about you.
- If you complete and submit an online information request form, we capture that identifying information and use it to fulfill your request.
With few exceptions (see below), the Plan does not share with a third party any personal information you enter in a form on our web site that might identify you, such as your full name, street address, telephone number, or email address.
Exceptions. We might release your personal identification information in these cases:
- We do sometimes share information that you enter with our trusted business partners, who have agreed to follow our privacy policies.
- We will release specific information about you to comply with any valid legal process such as a search warrant, subpoena, statute, or court order.
- We may also release specific information in special cases, such as if there is an attempted breach of the security of the Plan's web site, or a physical threat to you or others.
- We may also use information you give us to help improve our site's usability.
Your privacy in online forums
In the future, we might ask for personal identification information from you if use any new online services, such as:
- Surveys
- Forums
- Subscription registrations
- Bulletin boards
- Discussion groups
- Requests or suggestions
In all of these cases, we will collect only the personal identification information you choose to give to us. We will allow you to review and change such personal identification by contacting the Plan.
Please note: If you give out personal information online — such as on a bulletin board or discussion group — that information can be collected and used by other users of the bulletin board or discussion group. Although we try to protect the privacy of our users, we cannot guarantee the security of information you post in such forums. You disclose such information at your own risk.
Cookies and your privacy
A cookie is a small data file that certain web sites save to your computer when you visit them. Cookie files are often used to track the pages you visited. But the only personal information a cookie can contain is information you supply yourself. A cookie can't read data off your computer or read cookie files created by other sites.
