Rights and Protected Health Information
Community Health Plan cares about the protected health information rights of our members. Federal law gives members the right to:
- Access the protected health information that Community Health Plan has about them.
- Ask us to change the protected health information Community Health Plan has about them.
- Ask for a list of the times over the past six years when we shared the member's protected health information with another person or organization.
- Ask for certain restrictions about how we share the protected health information.
- Ask us to change the member's contact information.
- Get a copy of our Notice of Privacy Practices.
There are certain exceptions to these rights. These exceptions and other important facts about health information rights are explained in our Notice of Privacy Practices.
To use most of these rights, we will need a signed request from the member. To use his or her protected health information rights, the member will need to download and fill out these forms:
- Authorization to Release Health Care Information
- Request to Access Your Protected Health Information (PHI)
- Request to Correct Your Protected Health Information (PHI)
- Request for an Accounting of Disclosures of Your PHI

Use and Disclosure of Protected Health Information
Community Health Plan may share a member's protected health information as a part of our work to help the member get and pay for medical care. We must also share her or his protected health information with some people or organizations.

Security of Protected Health Information
Community Health Plan takes care to keep protected health information safe. We use technology to protect private health information. We check and evaluate our processes and systems to make sure PHI is protected.
We only give access to protected health information to certain people. If by mistake protected health information is shared with someone who shouldn't see it, we take steps to correct the mistake. We will tell the member and the right government agencies if information is accidentally shared with someone who isn't required to protect it under the law.
Our office is physically secure. We control access to our office with security access procedures and require that all people who enter our facility wear identification.
Our staff is trained to protect the privacy and security of member health information. Our staff protects information about members in a number of ways:
- We don't discuss private health information where others can hear it or with anyone who doesn't need to know it.
- We limit what we discuss on the phone.
- We keep written health information locked in a drawer when we aren't using it.
- We send health information by email in a form that can't be read if somebody else sees the email.
- We keep our computers locked when we aren't at our desks.
We check to make sure our staff follows these rules and we correct those who break them.

Community Health Plan Policies and Procedures
Community Health Plan has policies and procedures in place to protect the privacy and security of health information. The same protections are also required of any employer or plan sponsor who gets personal health information from us.
