Community Health Plan of Washington has created a program that rewards you for caring for your baby before they’re born.
$60 Gift card for the first visit
Get a $60 gift card for Amazon, Safeway, or Target when you visit your doctor during your first trimester.
$40 Gift card for the second visit
Get a $40 gift card for Amazon, Safeway, or Target for a second prenatal visit.
How it works
- Be a CHPW member.
- Complete one prenatal visit in the first trimester or within 42 days of becoming a CHPW member.
- Fill out the Online Reward Submission Form within 180 days of that visit to qualify.
- You will receive a $60 gift card in the mail within two to three weeks.
- Complete a 2nd prenatal visit. Then fill out the Online Reward Submission Form within 180 days of your 2nd visit to qualify for a $40 gift card.
