Children First: Great incentives for patients, easy for providers
Since we launched our Children First™ incentive program earlier this year, more than 3,600 of our members have gained useful health and safety equipment simply for receiving routine-but-essential health care. Members receiving prenatal care, well-child exams, and immunizations have taken home brand-new car seats, booster seats, backpacks, thermometers, and more.
To get their incentives, members need signed forms from their health care providers. Knowing how busy providers are, we've made that process as simple as possible: Just fax one short form after you've treated the eligible patient, and his or her reward will be on the way.
We hope you'll encourage your patients to take advantage of Children First. To help, we've provided the following answers to frequently asked questions about the program.
Who is eligible for Children First?
All Community Health Plan members, regardless of provider.
Are members eligible for only one incentive?
No. Members may be eligible for multiple incentives if they seek the required care and remain members of Community Health Plan. For example, if a member seeks the full course of prenatal care, she could be eligible for a car seat. Then if she remains a member and her child becomes a member who remains current on his or her immunizations, the child could be eligible for a number of other incentives.
How do patients receive enrollment forms?
From their providers. All primary care providers in the Community Health Plan network should have a supply of Children First prenatal and well-child/immunization enrollment forms. For more forms (available in English and Spanish), please e-mail or call your Community Health Plan provider relations coordinator.
What is the best way to submit forms on behalf of patients?
It's easy—just fax the completed form to Community Health Plan once the patient receives the required care.
Can a nurse sign the eligibility form instead of a provider?
Yes. We can accept forms from a nurse or any other permanent staff member who is involved with the member's care.
What is required to complete the prenatal program?
A member must see her provider for prenatal care within the first trimester and still be seeing her provider for prenatal care in the third trimester of her pregnancy.
Who is eligible for the well-child program?
All Community Health Plan members, from infancy until one day before their 14 th birthdays. Eligibility also requires that they must be current or in the process of becoming current on scheduled immunizations.

Please note: Car seats are not available to Community Health Plan members insured through the Public Employees Benefits Board (PEBB), and incentives may be limited to a maximum dollar amount each year as established by Washington State law.
