Clinical Guidelines
Resources for Providers: Guidelines for Chronic Medical Conditions and Preventive Services: Community Health Plan uses guidelines for the chronic diseases (including two behavioral health conditions) listed below. Reference is made to the pertinent evidence-based, peer-reviewed guidelines from nationally recognized agencies. This list is reviewed annually by Community Health Plan Medical Directors and by practitioners in the appropriate scopes of practice from the Community Health network. Additional review by affiliate or other providers is obtained if needed. Paper copies of these links and of the guidelines themselves are available from Community Health Plan on request.
