Patient Navigators
The Patient Navigator Program is part of the Community Health Plan/Community HealthFirst Medicare Advantage Special Needs Plan. The Patient Navigator is a patient advocate who conducts a comprehensive health risk assessment and formulates an action plan which is shared with the primary care provider. Their services may include:
- Help finding and making appointments with primary care providers
- Help finding and making appointments with specialty providers
- Answers to questions about managed care features such as benefits, referrals, referral authorizations and the Part D formulary
- Help understanding Medicaid wraparound coverage
- Help finding and making appointments with dentists who accept Medicaid reimbursement for the Medicaid dental benefit
- Help securing non-emergent transportation from Medicaid's transportation brokers
- Assistance with maintaining Social Security Administration and Medicaid dual eligibility paperwork
- Assistance in using translation services when appropriate
- Assistance in accessing local volunteer donation services and other community resources
