Paper Claims - CMS-1500 (08-05)

While the Centers for Medicare and Medicaid Services (CMS) is set to reject all paper claims on the old HCFA form (CMS 1500 – 12-90) beginning on July 2, 2007, Community Health Plan will continue to accept the old form for a short period of time.

Community Health Plan strongly recommends that providers start using the “New” CMS-1500 (08-05) form immediately to conform to CMS guidelines .
