Keeps on Giving
Community Health Plan wants to keep your children safe and healthy. That’s why we have started sending special birthday postcards to children when they turn 6 months, 9 months, 15 months, 18 months, and 3 years old. The postcards are a reminder to schedule well-child exams and immunizations (shots).
Community Health Plan knows how busy parents are. We hope the birthday postcards can help in planning for these important appointments.
