Free 24/7 Nurse Advice Line for all Community Health Plan members
All Community Health Plan members can now call the free Nurse Advice Line to get health care information 24 hours a day, 7 days a week. To speak to a nurse, call toll free 1-866-418-1002 (voice) or 1-866-418-1006 (TTY for speech or hearing impaired).
The nurses can help you when you have questions about health concerns or need health information.
Nurse Advice Line staff can help you decide what kind of care to seek. You might be able to treat the condition at home, you might need to see your doctor, or you might need to get urgent or immediate care. If you have a life-threatening medical emergency, call 911 or your local emergency number.
The Nurse Advice Line staff will also let your provider know about your call and the advice you got.
Let the nurse know when you call if you need an interpreter.
Children First rewards Community Health Plan members for getting the important care you need
If you are pregnant and a Community Health Plan member who completes the Children First™ program, you can get a FREE car seat.
Children who complete the Children First program can get one or all of the following FREE items.*
- Children's books
- School backpack
- Baby welcome kit
- Booster seat
- Bike helmet
* The rewards may be different than shown here. Other requirements and restrictions may apply to the Children First rewards program.

Community Health Plan can help to support high-risk moms during pregnancy or postpartum recovery. If you need this support, case management will keep in touch with you by phone during the pregnancy and the 6-week postpartum recovery. We also work with an in-home nursing service that can:

If you are enrolled in Healthy Options and want to change to Community Health Plan, call DSHS at 1-800-562-3022.
Children's Health Insurance Medical Plan (CHIP)
If you are enrolled in CHIP and want to change to Community Health Plan, call DSHS at 1-800-562-3022.
If you want to enroll in Healthy Options or CHIP, visit the Community Service Office (CSO) near you or call the Help for Working Families Call Center at 1-877-980-9131.
Basic Health
